ABSTRACT

The regulation also sets out obligations on marketing authorization holders (MAHs) and medical devices manufacturers, authorized representatives, importers, distributors, and notified bodies duties to: * Provide information that will assist with the monitoring by the MSSG/MDSSG of shortages of medicinal products and devices contained in the critical medicines list and updating this when necessary * Provide a justification for any failure to provide the requested information by the established deadlines * Immediately provide any evidence of an actual or potential shortage of medicinal products or devices * Provide information to EMA by 2 Sep. 2022 that will enable the establishment of a Single Point of Contact (SPOC) network, and the industry Single Point of Contact (iSPOC) network, with mandates to updating this when necessary (7). Enhanced advisory role In addition to the measures designed to address medicinal and medical device shortages, an Emergency Task Force (ETF) will also be established within EMA to provide scientific advice and review evidence on medicines that have the potential to address public health emergencies, offer scientific support to facilitate clinical trials, and support existing EMA committees with their authorization and safety monitoring of medicines (8). According to the document published by EMA on Crisis Preparedness and Management, the key benefits for EMA of having these responsibilities include: * Accelerated evaluation and access to safe and effective medicines which could treat or prevent a disease causing, or likely to cause, a public health emergency * Improved quality of data and the effective use of resources through increased support at EU level towards the conducting of clinical trials in preparation for, and during a public health emergency * Improved coordination and harmonization at EU level in preparation for and during a public health emergency (7).

ABSTRACT

The antibiotic amoxicillin is usually recommended as a first-line treatment for many common infections affecting children. Repeated lockdowns related to the coronavirus disease 2019 pandemic have contributed to supply difficulties for many drugs, including antibiotics. However, the risks associated with amoxicillin supply shortages appear not to have been sufficiently assessed, and the crisis we are facing today is serious and particularly dangerous for children's health. Without rigorous measures to prevent shortages related to drug production and distribution, populations could face a post-antibiotic era in which common infections and minor injuries can result in serious life-threatening situations. The availability of medicines declared by WHO as essential should be guaranteed not only in production but also in fair distribution. ABSTRACT

Energy poverty is a multifaceted phenomenon that affects many Europeans. Alleviating energy poverty is high in the EU, national, and local policy agendas. Despite the attention the phenomenon has been gaining from a policy perspective, especially after the current energy crisis, there are still some gaps due to the complexity of the issue and its vastly different manifestations across Europe. This manuscript presents the policy implications stemming from the implementation of the POWEPROOR approach in alleviating energy poverty in eight European countries, as co-created with relevant stakeholders in each country. The knowledge gained from empowering energy-poor citizens by promoting behavioural changes and small-scale energy efficiency interventions, as well as by encouraging the uptake of renewable energy sources in the form of collective energy initiatives while leveraging innovative financing schemes, resulted in policy recommendations for national and sub-national governments and lessons for civil society and the private sector.

ABSTRACT

PurposeThis study explores workforce related adaptation by e-tailers during social distancing causing crises (SDCC) and provides a conceptual framework.Design/methodology/approachA grounded theory based approach has been used wherein about 120 news articles were analyzed to understand the real-world measures taken. 50 journal papers were also referred to. A typical qualitative methodology, including open, and axial was used.FindingsIn the early stages, panic buying emerged as the key disrupting factor which necessitated staff shortage management. In the long term, e-tailers can adapt to prevent reverse worker migration and modify their hiring and training processes.Research limitations/implicationsThis study synthesizes knowledge on workforce-related adaptation by e-tailers and offers considerable potential for future research as well as the development of case studies and consulting services for the industry. Two research propositions are offered that can guide hypothesis generation and further studies can be conducted in sectors other than retail also.Practical implicationsThis study puts forward propositions based on theoretical dimensions for managers adapting to workforce-related problems during SDCC. The pandemic has led to vast unemployment and the shutting down of a number of businesses across the globe due to economic downfall. Hence, this study has economic and social implications.Originality/valueThis study is unique as it is one of the few that delves into e-tailers' workforce- related adaptation as SDCC evolves and contributes to a body of literature which is scarce.

The WHO deemed that interprofessional education and practice (IPEP) is vital to attain a workforce that Is ready and able to care for local health needs through teamwork and collaboration (WHO. 201 Oh Interprofessional teams are critical in the care of acute, chronic, and complex health and social support needs of COVID-19 patients (Michalec & Lamb. 20201.1 PE and IPP are so essential to care that accreditation bodies have come together to provide guidance about how to embed these activities throughout all health professions programs (Health Professions Accreditors Collaborative. 2019). ABSTRACT

The large consumption of fast fashion brings many negative environmental impacts. Filipino consumers love and buy fast fashion because it is relatively cheap but trendy, and there are lots of fashionable designs to choose from. Despite the shortage in water supply and disposal issues of fast fashion, people still continue to purchase. The lack of awareness of consumers on sustainable fashion consumption led the researchers to conduct a study that aims to identify factors affecting Filipino consumers' buying decisions on fast fashion using the combined theory of planned behavior, elaboration likelihood model, and hedonic motivation. A total of 407 participants were gathered through a convenience sampling approach, and the data collected were analyzed using structural equation modeling (SEM). The result shows that attitude towards fast fashion is the highest contributing factor to purchase intention. While social media positively affects purchase intention, sustainability advocacy negatively impacts the consumers' intention to buy fast fashion. The awareness of sustainability leads to consumption reduction of fast fashion garments. Surprisingly, perceived product price and quality do not show a significant influence on purchase intention. Incorporating sustainability advocacy on social media may be a great strategy to encourage the sustainable consumption of fashion garments. The findings of this study could be a great tool to influence fashion companies and government institutions to promote sustainability awareness and transition marketing strategies to the sustainable consumption of fashion.

ABSTRACT

BackgroundEarly 2020, short supply of hydroxychloroquine sulfate (HCQ) – then claimed as effective for treatment of COVID-19 in several countries - raised significant concerns for those who use HCQ for chronic diseases including SLE. In April 2020, Lupus Europe launched a survey to quantify the access gap, as well as the anxiety expressed by patients confronted with this outage. 2422 patients responded with an average level of anxiety about access to HcQ of 6.45 on a maximum of 10. After supply issues were resolved, a follow up survey (Aug-Sept 2020 – 1854 answers) showed a significantly reduced anxiety of 4.15. A second follow up survey (Nov 2021 -Feb 2022 - 2255 answers) showed a further reduction to 3.54. Importantly, a core group of 6.2-13.7% of patients remains extremely anxious (scoring 9 or 10) about supply of their medication, substantially more than in Portugal (3.5%) and Finland (0%), where supply remained fair at the heat of the crisis.ObjectivesTo provide consensus on how communication should be conducted to minimize the impact of medicine shortages on patient anxiety.MethodsThe Patient Advisory Network (PAN) of Lupus Europe established an extensive list of potential elements of a more effective communication on shortages, that could help reduce patient anxiety. 20 statements were derived from those elements and proposed to PAN members and Lupus Europe member organisations. 101 answers were collected from 17 countries. For each country, the individual ratings of all participants were averaged to assign an individual vote for each country on each statement. Consensus amongst the 17 countries was then considered as obtained if 14 or more of the 17 countries agreed or strongly agreed with statements.Results9 out of the 20 statements reached consensus:1. Lupus specialists should (a) clarify alternative medication existing and its difference versus current treatments, (b) clarify appropriate emergency procedures and (c) clarify to the patients how to handle a short supply.2. Specialists and Hospitals should establish alternative supply mechanisms to guarantee minimum availability.3. pharmaceutical industry should (a) provide all information to all stakeholders and (b) help create emergency supply routes to ensure that no patient is left without his/her medication for a sustained period of time.4. National authorities should help patients with demonstrated need have priority access to limited supply quantities through a simple process.5. Hospitals should communicate (by email or postal mail) information on the shortage, how to handle it and how to access emergency supply routes. The same number agreed with Health authorities performing that same communication.6. pharmaceutical industry should avoid diverting products from one country to another if that would reduce supply below normal consumption level.7. Patient organisations should stop the rumors that can quickly spread though social media.8. pharmacists should be better equipped in terms of data (on the reasons and clear timelines for resolution as well as alternatives or recommendations for patients facing shortages)9. GP's should clarify alternative medication and appropriate emergency procedures.ConclusionShortages of medicine create an anxiety that can be long lasting. Even when supply is re-established, the fear remains. For this reason, establishing an effective communication system is necessary to reassure patients when short term shortages are taking place, and is key to avoid fast spreading anxiety relating to this concern. ABSTRACT

BACKGROUND: Staff shortage is a long-standing issue in long term care facilities (LTCFs) that worsened with the COVID-19 outbreak. Different states in the US have employed various tools to alleviate this issue in LTCFs. We describe the actions taken by the Commonwealth of Massachusetts to assist LTCFs in addressing the staff shortage issue and their outcomes. Therefore, the main question of this study is how to create a central mechanism to allocate severely limited medical staff to healthcare centers during emergencies. METHODS: For the Commonwealth of Massachusetts, we developed a mathematical programming model to match severely limited available staff with LTCF demand requests submitted through a designed portal. To find feasible matches and prioritize facility needs, we incorporated restrictions and preferences for both sides. For staff, we considered maximum mileage they are willing to travel, available by date, and short- or long-term work preferences. For LTCFs, we considered their demand quantities for different positions and the level of urgency for their demand. As a secondary goal of this study, by using the feedback entries data received from the LTCFs on their matches, we developed statistical models to determine the most salient features that induced the LTCFs to submit feedback. RESULTS: We used the developed portal to complete about 150 matching sessions in 14 months to match staff to LTCFs in Massachusetts. LTCFs provided feedback for 2,542 matches including 2,064 intentions to hire the matched staff during this time. Further analysis indicated that nursing homes and facilities that entered higher levels of demand to the portal were more likely to provide feedback on the matches and facilities that were prioritized in the matching process due to whole facility testing or low staffing levels were less likely to do so. On the staffing side, matches that involved more experienced staff and staff who can work afternoons, evenings, and overnight were more likely to generate feedback from the facility that they were matched to. CONCLUSION: Developing a central matching framework to match medical staff to LTCFs at the time of a public health emergency could be an efficient tool for responding to staffing shortages. Such central approaches that help allocate a severely limited resource efficiently during a public emergency can be developed and used for different resource types, as well as provide crucial demand and supply information in different regions and/or demographics.

ABSTRACT

OBJECTIVES: To examine whether facility-reported staff shortages and total staff levels were independently associated with changes in nursing home (NH) outcomes in 2020. DESIGN: Longitudinal cohort study. SETTING AND PARTICIPANTS: A total of 8466 NHs with staffing and outcome data. METHODS: This study used NH COVID-19 Public File (2020), Nursing Home Compare (2019-2020), and Payroll-Based Journal data (2019-2020). Outcome measures included the percentage of long-stay residents in a facility with declines in activities in daily living (ADLs), decreases in mobility, weight loss, and pressure ulcers in 2020 Q2, 2020 Q3, and 2020 Q4. Independent variables were whether NHs reported any shortage of aides or licensed nurses and total staff hours per resident day (HPRD). Separate 2-level (NH, state) Hierarchical Generalized Linear Mixed models examined the association of facility-reported shortages and staff hours with key NH resident outcomes, controlling for NH characteristics and COVID-19 infections. RESULTS: The weekly percentage of NHs reporting any staff shortage averaged 20%. Total staff HPRD increased slightly from 3.7 in 2019 to 3.8 in 2020. Health outcomes were stable during 2019 and 2020 Q1 but worsened substantially starting in 2020 Q2. For example, the percentage of residents with mobility loss increased from 16.2% in 2020 Q1 to 27.9% in 2020 Q4. Facility-reported staff shortages were associated with an increase in the proportion of residents with an ADL decline (0.54 percentage points), mobility loss (0.80 percentage points), weight loss (0.22 percentage points), and pressure ulcers (0.22 percentage points) (all P < .01). Total staff HPRD was not associated with changes in any outcomes (all P > .05). CONCLUSIONS AND IMPLICATIONS: NHs reported worsened health outcomes among long-stay residents in 2020, with worse outcomes found among facilities that reported staff shortages but not among those with lower total staff levels. Facility-reported shortages provide important quality information during the COVID-19 pandemic.

ABSTRACT

PurposeOrganizational culture has been identified as an important factor in increased employee commitment. Particularly during a shortage of skilled workers, commitment is a meaningful indicator of higher loyalty and retention. However, limited research has studied the relationship between organizational culture and commitment from a global perspective. Most research focuses on specific aspects of culture and examines the aspects' effects on commitment separately. The author's objective is to identify influential organizational culture's dimensions and assess dimensions' relationship to commitment holistically.Design/methodology/approachFor this purpose, the author analyzed a data set with 241,648 participants from 9 industries in Germany. The survey captures individual attitudes toward certain aspects of organizational culture and assesses workers' commitment to their organizations.FindingsThe results of a linear regression show that all cultural dimensions considered, namely transformational leadership behavior, team cohesion, compensation, fairness and caring attitude, if well-developed, positively and significantly influence organizational commitment. Interestingly, team cohesion has the greatest effect on commitment, followed by transformational leadership behavior, compensation, caring attitude and fairness.Originality/valueThis paper aims to examine the relationship between organizational culture and commitment holistically, thereby revealing which aspects of corporate culture are particularly important for increasing workers' commitment.

ABSTRACT

Stormwater harvesting via managed aquifer recharge in retrofitted infrastructure has been posited as a method for resource augmentation in Cape Town. However, the existing guidelines on stormwater retrofits are technically inclined, occidental, and generally misaligned with the realities and socio-economic contexts of developing nations like South Africa. Water and urban practitioners from developing nations cannot just 'copy and paste' existing guidelines as different socio-economic dimensions and colonial histories typically hinder 'traditional' approaches. This paper assesses how a transdisciplinary team navigated these realities in a case study of a retrofitted pond in Mitchells Plain, Cape Town. A decolonial thinking framework was applied for reflection and thematic content analysis. The framework was used to unpack how the team encountered, addressed, and learned from the challenges during the retrofit process. The research team found that the retrofit process within a context of under-resourced South African communities can be viewed as developmental work with a strong emphasis on continuous community engagement. Thus, it is suggested that in the South African context, water practitioners should consider, at the fore, interaction with local communities, including awareness of racialised histories, to ensure projects are successfully implemented and completed.

ABSTRACT

Through a detailed analysis of the visual imagery as well as the verbal mode of narration, in Sarnath Banerjee's Graphic narrative All Quiet in Vikaspuri, the study suggests that the water-deprived, post-apocalyptic world that Banerjee reflects, is a spitting image of the Anthropogenic water crisis in India. Drawing theoretical insights from Madhav Gadgil, Ramachandra Guha and Dipesh Chakrabarty, the paper attempts to suggest the "Great Indian Water Crisis" is fueled by "short-termism," increased corporate privatization of water, myopic government development policies and erection of dams and other capitalist structures. The paper also aims to uncover how sociopolitical "slow-violence" is rendered to the natural resources under the garb of "Vikas" (development) and privatization. By contriving the narrative around the quest for the river Saraswati, Banerjee draws attention to the ever-so-real issue of groundwater overextraction in India, leading to its dipping levels and in turn, depletion. Further, the paper argues that "intermediality" of graphic narration abets Banerjee to cater to "the representational challenges" of the Anthropocene.

ABSTRACT

Australia launched a Seasonal Worker Programme (SWP) in 2012, shortly after a similar scheme in New Zealand, to bring seasonal workers from Pacific Island Countries (PICS) to work in agriculture. The scheme was seen as a potential ‘Triple Win' with sending and receiving countries, and workers' households, benefiting. Workers' remittances contributed to welfare, especially housing and education, and small business establishment, but there were social costs associated with repeated absences. In 2018, Australia introduced the Pacific Labour Scheme (PLS) to extend guestwork opportunities into other areas of non-seasonal labour shortage such as aged care, tourism and meat processing. The shortage of local labour during COVID-19 demonstrated that Pacific guestworkers were invaluable to Australia, and in 2022 the schemes were revamped and expanded further as the PALM (Pacific Australia Labour Mobility) Scheme. Concern over a Chinese threat in the region gave further support for the expansion. PICs expressed concerns about exploitative practices, while higher rates of participation increased the potential for an incipient brain drain from the PICs, with wages roughly four times those at home, as migrants now left non-agricultural jobs. The expanded scheme continues to favour Australian employers leaving questions over, equity, uneven development and the future of the PICs.

Data provided to Kaitiaki, after a complaint to the Ombudsman, showed the demand for additional shifts did not evaporate, as predicted. Mccallan said winter, and related illnesses, had always increased pressure on health services - but it had just got a lot worse as a result of the crisis of short-staffing.

Laboratory medicine and the services provided throughout the healthcare system remain critical components in the ability to deliver quality patient care. Over the years and as of recently, due to COVID-19, delivery of proper care has been challenged with continual staffing constraints within the clinical laboratory. While this is not a new issue, its exacerbation poses an ongoing threat to clinical laboratories nationwide and has many healthcare administrators and executives searching for viable solutions. This paper focuses on exploring the depth and breadth of the issue to get to the root of the cause(s) of the critical shortage. Once these keys issues are identified, then realistic and effective solutions can be proposed and implemented throughout the laboratory field.

ABSTRACT

Purpose: the purpose of the research is to study the limited possibilities of financing projects in the field of industry 4.0 and the fight against climate change in the regions of Russia in the face of budget shortages amid the pandemic and the COVID-19 crisis, as well as to determine the optimal budget conditions for financing these projects. Design/methodology/approach: in order to determine the most accurate and reliable dependencies of the volume and financing of projects in the field of industry 4.0 and the combat against climate change on the balance of the regional budget, this research is carried out on the basis of two samples of regions of Russia, formed according to the criterion of the regional budget balance in the context of a pandemic and COVID-19 crisis in 2020: with a budget surplus and deficit. Findings: the results of the research showed that the financing of projects in the field of industry 4.0 and the fight against climate change in the regions of Russia, firstly, is significantly determined by the balance of the regional budget. Secondly, it is characterized by various laws: financing of projects in the field of industry 4.0 is scarce both with a deficit and a budget surplus, and financing of projects in the field of combating climate change in the regions of Russia is scarce with a budget deficit and enough with a budget surplus. Originality/value: differences in the conditions of optimal budget balances are justified: with a zero regional budget balance, the point of optimal (deficit-free) financing of these projects is reached, and a confident regional budget surplus forms the zone of optimal financing of climate change projects in the regions of Russia. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2022.
